Investigation Summary
At 7:30 a.m. on October 13, 2017, an employee was troubleshooting the Model 310 Smetco High Speed Pallet Sorting Equipment. The employee placed a hand inside the machine to clean the photo sensors of the machine by wiping the sensors. The Smetco High Speed Pallet Sorting Machine moved from a horizontal position to a vertical position while the employee's hand was inside the machine with moving parts. The employee sustained fractured and lacerated fingers and hand. The equipment was not locked-out.
