Investigation Summary
At 8:15 a.m. on October 18, 2017, an employee was gathering a sample of sand from a conveyor belt for the testing laboratory. The employee reached with his right hand into the opening at the conveyor between the muller chute and the aerator when the conveyor started suddenly, pulling the employees hand into the aerator mixing shaft. The employee amputated his arm above the elbow and was transported to the hospital by EMS. The employee was hospitalized for six days.
