Investigation Summary
Employee #1 was in a steel material basket slid onto the forks of a forklift truck. Employee #1 climbed into the basket and was elevated approximately 8 feet above the ground. As he was drilling a hole, the basket slipped off the forks, and Employee #1 fell to ground and later died of injuries sustained in the fall.
