Employee killed in fall through roof opening

Employee #1, while working on a roof installing siding, stood on a roof opening 44-inches by 44-inches that was covered with a single piece of two-inch thick styrofoam hardback insulation. The insulation collapsed under Employee #1, causing him to fall 28-feet to the pavement, where he died.
