Employee Killed In Fall From Aerial Lift

Employee #1 was in an aerial lift moving overhead lines. The aerial lift platform was elevated 20 ft. above the floor and was positioned between a bus-bar and a continuous moving conveyor. A "dog" on the conveyor caught the guardrail of the lift, pulling the lift over and throwing Employee #1 to the floor. Employee #1 was killed.
