Investigation Summary

Investigation Nr: 201403862
Event: 05/11/2000
Employee injured in fall from roof

Employee #1 was installing insulation with tabs on the west wall, working from the north to the south. When he finished, he had the insulation bent over the top of a double purlin that ran for approximately one bay in the building. Employee #1 was walking on top of the double purlin back to the north end when it switched to a single purlin. When he reached this point, he stepped forward with his right foot, but there was no purlin underneath to land on. Employee #1 fell approximately 30 ft, striking his head on the concrete floor. He was transported by ambulance to Hurley Medical Center in Flint, MI.
