Investigation Summary
At 5:30 p.m. on August 22, 2017, an employee was struck by a forklift that was being operated by a coworker who was off-loading pallets of alcoholic beverages. Both of the employee's legs were struck by the forklift and crushed as the coworker backed the vehicle up. The employee was hospitalized with multiple fractures of both legs.
