Investigation Summary
Employees #1, #2, #3 and #4 were working on the third floor deck of a building under construction. Thirty-four wooden trusses, 69 ft long, collapsed and fell to the third floor deck trapping all four employees. Employee #1 was killed and Employees #2, #3, and #4 were injured. Employees #2 and #4 were hospitalized.
