Investigation Summary
On November 8, 2002, Employee #1, a temporary service employee, was working to load bales of recycled paper onto a conveyor with a forklift, and then cut the wire holding the bale together. The bales moved up the conveyor incline to the beater. Employee #1 was found dead about 15 ft on to the conveyor incline with a bale on top of him. There were no witnesses to the accident.
