Employee slips on ice shield and falls

On June 5, 2002, Employee #1 was installing tar paper on a roof at height of 20 ft. Employee #1 slipped on an ice shield and fell. Employee #1 was not wearing fall protection. Employee #1 was hospitalized with a fractured back.
