Employee Killed When Struck By Flying Fragments

Employee #1 was killed when he was struck by flying fragments. He finished performing repairs on a steam turbine. He placed the turbine in start up mode and began testing the turbine. During the second test, the turbine went into an over speed acceleration and began to fail. The rotor buckets propelled through the two-inch-thick housing striking him with flying fragments. Employee #1 died.
