Employee Died After Falling From Ladder

Employee #1 died after he was attempting to climb an 18-foot ladder onto a roof to remove snow. The ladder slid out from under Employee #1 causing him to fall to the ground. He struck his head and had concussion. He died 5.5 months later from his injuries.
