Investigation Summary
At 1:30 p.m. on October 14, 2017, Employee #1 and Coworker #1, employed by a crane services company, were working at a multiemployer construction project. They were at the top of a grain elevator and removing a deteriorated bucket elevator. The bucket elevator collapsed, and Employee #1 rode on the collapsing bucket conveyor as it fell 12 feet to the elevator platform. Employee #1 was caught and pinned between the platform's walkway railing and the collapsed bucket elevator. Emergency services were called, and Employee #1 was transported to the hospital. He was treated for a large laceration that extended around the left side of his torso from his back to his abdomen.
