Employee killed after struck on head by lumber

On January 18, 2002, an employee of Champion Steel, Inc., was dumping a large metal box container of slag into the back of a stake truck. While the box was up on its side for emptying, the employee used a scrap piece of lumber to lever the slag box. The slag box shifted and fell toward the employee causing the lumber lever to strike his head, and killing him.
