Employee Killed When Crushed His Head Inside Palletizer

On July 19, 2002, Employee #1 was off-loading a finished pallet from a machine. He walked to rear of the machine, used an Allen wrench to unbolt the guard at the back of the machine, and leaned inside. The machine indexed and sent out a finished pallet, and then was bringing in a new one. Employee #1's head got caught and was crushed. Employee #1 was killed.
