Investigation Summary
On October 16, 2017, Employee #1 and coworkers, employed by a roofing company, were working on the roof of a commercial building. Employee #1 was clearing away old roofing materials when he fell through an unguarded and unsecured skylight, a fall height of 40 feet. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a fractured skull and other head injuries, concussions, leg injuries, and back injuries.
