Investigation Summary
Two employees were working from an unsecured makeshift work basket which was lifted on the forks of a Pettibone rough terrain fork truck. The basket and the employees fell 24 ft to the ground. Employee #1 was killed and Employee #2 was hospitalized for bruises, contusions, and abrasions.
