Investigation Summary
Employee #1 died when he fell from a roof while using a "Better Spreader" applying roofing adhesive. He was near the roof edge and flipped the handle to stop the flow of adhesive. He died when he fell over the edge of the roof. Apparently, the warning line had been removed along that area to apply the adhesive.
