Investigation Summary
At 8:00 p.m. on October 7, 2017, an employee noticed that the grinder return bin had become clogged. The employee grabbed a broom handle that was kept in the area and pushed it inside the grinder return bin in an attempt to unclog the equipment. The employee stated that there was a noise, and then realized that the slide gate had closed while the hand was inside the grinder return bin, resulting in amputation of his hand.
