At 3:15 p.m. on October 9, 2017, an employee was cleaning a Sambazon Aci machine that was frozen and pushed a brush down into the machine. The employee had his right index finger pulled into the machine and his fingertip was amputated when the machine began operating. Hospitalization was not required.
