Investigation Summary
On April 26, 1999, Employee #1 was working from a 6-ft ladder installing a 23-ft-long wooden beam that weighed approximately 600 to 700 lbs. The beam fell during the installation striking Employee #1 across the chest. The beam knocked him off the ladder and struck him again after he fell. Employee #1 was killed.
