Investigation Summary
At approximately 11:20 a.m. on June 17, 1996, Employee #1, of the Walter Kieckhefer Company, was on a roof fixing an evaporator cooler when he slipped and fell 16 ft 1/4 in. through a nearby skylight to a concrete floor. He suffered massive head injuries, a fractured rib, and abrasions and contusions. Emergency help was summoned. Employee #1 was treated and then transferred to St. Luke's Hospital in Tempe, AZ, where he died at 12:20 p.m. the same day.
