Investigation Summary
At approximately 11:30 a.m. on April 17, 1996, Employee #1, of Chucky's Back, an excavation contractor, was working in an 8 ft deep by 18 to 20 in. wide by approximately 124 ft long trench. The east wall of the trench collapsed, burying him up to his mid-chest/shoulder level. The Apache Junction Fire Rescue Squad removed Employee #1 from the trench and transported him to the hospital, where he was treated and released the same day. The trench was not shored.
