Investigation Summary
At approximately 3:05 a.m. on December 18, 1996, Employee #1 was at work station #2 in Bay C of Building 15, loading sodium azide discs (i.e., grains) into a stainless steel staging tube pallet. There was an explosion and flash fire, and several of the discs in Employee #1's hands caught on fire. She suffered burns to her hands and face, and respiratory irritation. Employees #2, #3, and #4, who were working nearby, were taken to the hospital, treated, and released. This work station was part of the assembly line for manufacturing the inflator bodies used in automobile passenger-side airbags.
