Investigation Summary
On August 16, 1995, Employee #1 was operating a punch press. He placed his right hand into the point of operation and activated the punch press cycle at the same time. The punch press cycled and amputated the top half of his right index finger and right middle finger and cut his right middle finger. On August 31, 1995, Employee #2 was operating the same punch press and placed his right hand into the point of operation and cycled the machine, causing amputation to the tip of his right index finger.
