Investigation Summary
Employee #1, a truck driver, was strapping down loads of cartons on a pallet that weighed 2,140 lb. As he did so, the forklift operator continued loading other pallets onto a flatbed. Employee #1 was 2 to 3 ft from an elevated pallet that was about to be placed on the flatbed when the pallet broke. The cartons toppled and struck Employee #1on the head and upper body. He was crushed and killed.
