Investigation Summary
At 5:15 p.m. on September 29, 2017, an employee was cleaning out a hot pit using a metal rod with a sharp end and began chipping at clumped material. The metal rod was pulled into the conveyor and the employee and coworker tried to remove the metal rod by running the auger in reverse. The employee pinched his left hand between the tool and metal grate covering the pit. The employee amputated his pinky fingrt, lacerated his ring finger, and bruised his hand. The employee was hospitalized.
