Investigation Summary
At 7:40 a.m. on August 29, 2017, Employee #1 was standing on the fourth rung of a six foot ladder, using a cordless drill to remove a screw that was was holding a roll-up map. He was using his left hand to brace the map on the other end. As he removed the first screw, the weight of the map shifted, and startled Employee #1. He lost his balance and fell off of the ladder, fracturing his heel. Employee #1 was hospitalized for his injury.
