Investigation Summary
At 1:30 p.m. on October 5, 2017, an employee was setting a bundle of soapstone slabs on one of the client's A-Frame racks stationed next to the truck mounted crane. The employee disengaged one end of two 19 foot 2 inch polyester fabric slings from the crane hook and pulled the slings out from under the load. The employee was struck by and crushed under the slabs weighing approximately 6,481 pounds when they fell. The employee suffered crushing injuries to his pelvis and legs which resulted in the amputation of his right leg. The employee was transported to and treated at the Santa Rosa Memorial Hospital in Santa Rosa, California.
