Employee's hand fractured in molding machine

Employee #1 was operating a plastic molding machine. He was standing on a three-rung stepladder, reaching across the top of the machine to secure or fix a leaking water line when the core mold cycled, crushing and fracturing his right hand. Employee #1's decision to perform the task using a stepladder bypassed the manufacturer's lockout instructions, and the employer's lockout/tagout procedures for performing maintenance.
