Investigation Summary
Employee #1 and a coworker were installing an overflow drainage pipe across a public road. They had begun to excavate a 24 ft long by 5 ft 10 in. wide by 8 ft deep trench, with the coworker operating the backhoe and Employee #1 guiding his grade, when a piece of asphalt broke off and fell into the trench. Employee #1 jumped in to remove the asphalt when the trench caved in at one spot, covering him up to his armpits. The coworker radioed his main office for help and began to manually dig out Employee #1 so he could breathe. Employee #1 was transported by Life Flight to the Latter-Day Saints Hospital in Salt Lake City. He sustained fractured ribs, punctured lungs, a bruised liver, a bruised heart, and spinal injuries. His left kidney also had to be removed.
