Investigation Summary

Employee #1 was preparing to replace the roof cover on a small, flat 9 ft long by 4 ft 3 in. wide roof over an entryway. He was using a pry bar to remove the metal flashing when he stood up and took a step backward. His foot went over the edge of the flat surface and onto the asphalt shingles on the side of the roof. The shingle broke that he stepped on broke, causing his foot to slip. He fell 10 ft to the concrete sidewalk, fracturing his leg. A safety monitoring system was in place and a coworker, who was the site foreman and safety monitor, was approximately 30 ft away on the flat roof of a garage. He had Employee #1 in full view but the accident happened so quickly he was not able to warn Employee #1 as he started to back up.
