Employee's fingers crushed in rolls of beading machine

At approximately 9:45 a.m. on January 26, 1999, Employee #1 was rolling couplers through a bead forming machine when his right hand became caught in the rollers. His fingers were crushed and his ring finger had to be amputated. Employee #1 had been properly trained to safely operate the machine.
