Employee injured when torso caught in blow mold machine

At 8:00 p.m. on December 13, 1998, Employee #1, a maintenance technician at Infiltrator Systems, was investigating an obstruction in the air injection line of the blow mold machine. To access the line outlet, he went past the interlocked sliding barrier guard doors and positioned his body between the high pressure hydraulic molds. He then instructed the coworker operating the machine, who was approximately 10 ft away and within sight and voice contact, to depress the air control button on the control panel to feel if air was escaping from the outlet. The coworker accidentally depressed the "mold close" button, causing the molds to move to within 8 in. of one another. Employee #1's upper torso was crushed between the molds, fracturing his ribs and piercing his colon. A rag on a stick is normally placed in front of the air outlet to visually determine if air is escaping. The employer stated that this was the first time a worker had entered the machine for this purpose. Employee #1 had been with Infiltrator Systems for 2 years, 7 months at the time of the accident.
