Investigation Summary
Employee #1, age 16, was using a Delta table saw to rip a 1 by 4 pine board. He had placed the board against the fence and was supporting it with his left hand. Employee #1 was wearing a pair of Jersey gloves and, as he passed the board through the blade and pulled his hand back, his gloves became caught on the blade. His left hand was pulled in, lacerating his thumb and his index and middle fingers. All three digits were severed through the bone but not completely detached. A customer who was standing nearby notified other workers, who called 911. An ambulance took Employee #1 to Jordan Valley Hospital, where his fingers were surgically re-attached. Employee #1 had been on the job for just over 3 days. No formal training was documented, and he stated that the only training he was given was by a coworker, who also taught him about the radial arm saw. This was an assigned duty and Employee #1 was aware of the job's requirements. He had previous experience with woodworking tools from his high school shop classes. He also admitted that he was not using the guard even though it was available on the machine.
