Investigation Summary
At 3:44 p.m. on August 28, Employee #1 was sweeping the floor with gas exiting a propane tank he was holding. The gas met with an undetermined ignition source and an explosion occurred. Employee #2 was entering the room as the explosion occurred. Employee #1 sustained second and third degree burns to the upper body and face. Employee #2 sustained first and second degree burns to the face and arms below the elbows.
