Investigation Summary
Two medical facility maintenance employees were working along and near the edge of the roof on the South "D" Wing at the McKay-Dee Center while power washing the area in preparation for applying a paint sealant. Neither employee was provided with, nor did they utilize fall protection to prevent a fatal fall of 60 feet that occurred when Employee #1 either stepped, slipped, or tripped over the edge of the roof.
