Employee killed when caught in horizontal mixer

Employee #1, a machine operator for Wilson products, was using a horizontal mixer. She was working in the normal operating position when she either fell or was pulled into the mixer and became pinned inside. Maintenance workers tried to get her out, but eventually three of them had to manually pull the mixer blades backward to free her. Employee #1 died of asphyxia. There were no witnesses to the accident.
