Employee's fingers fractured when caught in tail pulley

Employee #1 was servicing a waste chip conveyor at a Frito-Lay facility. She was using a pressurized air hose to force out debris that had accumulated around a running conveyor when the hose became caught in the running belt. Employee #1 was trying to retrieve it when her right hand became caught in the tail pulley. She fractured two of her fingers and was transported to Pioneer Valley Hospital for treatment. The conveyor was not locked out or tagged out.
