Investigation Summary
At 1:15 p.m. on September 29, 2017, an employee was working at the Mega Cutter Line which was located between two Tile Turning Conveyors. The employee was clearing a tile back up in the beveling area by placing the the tiles on the floor. The employee fell forward after tripping on the tiles he just placed on the floor. As the employee attempted to catch himself, his left index finger was caught in conveyor belt that turns the tiles around for side trimming and amputated his middle finger. The employee was hospitalized.
