Investigation Summary
At 3:30 p.m. on September 25, 2017, an employee was troubleshooting a chocolate pump. The employee examined the machine and saw that the air operated actuator valve was stuck and reached into the machine to correct it. A coworker clicked the on/of switch and the actuator cycled, amputating the tip of the employee's right index finger.
