Employee's foot amputated in auger

At approximately 9:00 p.m. on August 16, 1996, Employee #1, age 19, was cleaning up around a grizzly covering an auger that transported material into the plant. One of the metal bars was missing from the screen grate. When Employee #1 stepped on the grizzly, his left foot went through into the auger and was amputated.
