Investigation Summary
At approximately 2:00 p.m. on June 10, 1996, Employee #1 was working on a highway overpass near Delle, Utah. He was being lifted by a crane while standing in a trash bucket. The crane malfunctioned and the bucket fell, along with Employee #1. The bucket bounced when it hit the end of the cable, inflicting injuries to Employee #1's head and legs. Neither the crane nor the bucket were designed to carry people.
