Investigation Summary

Investigation Nr: 171054117
Event: 04/03/1996
Employee killed in trench cave-in

At approximately 5:30 p.m. on April 3, 1996, Employee #1, age 19, of Landmark Construction, was installing 4 ft sewer laterals in the bottom of a 7 ft trench. The hardpan west side of the trench collapsed, crushing Employee #1 against the east side. Employee #1's supervisor and a coworker immediately began to dig him out; when freed of the debris, he was unconscious, unresponsive, and not breathing. CPR was started, and a coworker called 911 on a cell phone. Both fire and medical personnel responded and began administering emergency care. Employee #1 was transported to McKay Dee Hospital, where he was pronounced dead on arrival.
