Employee's foot fractured when run over by forklift

At 6:00 a.m. on September 19, 1995, Employee #1, age 18, of Clover Club Foods, was operating a Clark stand-up battery operated forklift at the Clearfield Distribution Warehouse. While driving in reverse, he stepped off the back of the lift. The rear wheel caught and ran over his left foot, fracturing it. Employee #1 was taken to the North Davis Medical Center.
