Investigation Summary
At approximately 3:30 p.m. on August 25, 1995, Employee #1, a maintenance assistant, and a coworker, a maintenance lead man, were performing a routine maintenance procedure on an American HY-PRO towel folding machine. This involved removing a cover, exposing a chain and sprockets, and adjusting and lubricating the chain. All power to the machine had been shut off. At some point, the coworker was called away and Employee #1 was left to finish the job alone. He decided to restore power and activate the towel folder, causing the chain and pulley system to engage. The employee was oiling the chain by holding an oil can in one hand and in the other was a rag that he had wrapped around the chain to catch the excess oil. The rag pulled his hand into one of the sprockets, resulting in a deep laceration at the base of the right index finger (at the proximal end of the second metacarpal) and a fractured bone, but the finger was not amputated. Employee #1 had been trained in lockout procedures and admitted to not following company policy. The power to the machine was shut off via a lever on the control box, but the lever was not locked with a padlock. No other employees were in the vicinity.
