At approximately 2:30 p.m. on or about August 12, 1996, Employee #1, of Tualatin Valley Building Supply, Inc., Salem, OR, was using a table saw to cut plywood sheets into 6 in. by 48 in. strips. His right hand struck against the saw blade, amputating his thumb, index, and middle fingers.
