Investigation Summary
At 3:30 a.m. on September 2, 2017, an employee was attempting to clear a jam from a Baader fish filleting machine. The employee asked a coworker to turn the machine off. The employee was told the machine was turned off, but when he reached his hand inside to clear the jam, the belly blade was still running. The blade cut his right index finger resulting in overnight hospitalization and surgery.
