Investigation Summary

Employee #1 had turned off all driven equipment and was clearing a jammed sheet of plywood from the end trim saws, when he presumably lost his balance and fell backward across an opening between the outside edge of a trash conveyor and a sheet turner crossbar. His head and neck hit the crossbar, rendering him unconscious. He then went through the approximately 3 ft 6 in. by 2 ft 11 1/2 in. opening and fell approximately 5 ft 2 in. to the floor. He was transported from the Redmond Plant to St. Charles Medical Center, Bend, OR, where he is in the critical care unit with a major head injury. He remains unconscious at the time of this writing.
