Investigation Summary
At 7:00 a.m. on April 17, 1998, Employee #1 was using a pee vee to straighten a log on the log transfer deck to the debarker. After several unsuccessful attempts to move the log into correct alignment the log contacted the inclined conveyor on one end which caused it to strike Employee #1 in the chest knocking him backward onto the debarker infeed conveyor. Employee #1 was hospitalized with contusions and abrasions to his chest and back.
